At TRU Form Skin Lab booking is done online! Follow these simple steps to "Discover your Tru Form". 1. Go to the "Treatments" page, scroll through (click the treatment for more information) 2. Click “BOOK NOW”, choose your preferred date and time. 3. Securely enter your name, email, phone number, and payment information. 4. Pay your $50 deposit. The deposit is used to hold your appointment and will be used a credit toward your scheduled treatment. If your appoinment is cancelled 24 hours before your scheduled treatment, you will be refunded*. 5. You will receive an appointment confirmation email immediately, and another detailing how to prepare for your treatment. 6. You can also call to book Monday through Sunday from 10am until 6pm. *In an effort to avoid late cancellations and no shows we have instituted a new policy: If your appoinment is cancelled with less than 24 hours notice, the $50 will not be refunded, and cannot be used toward future treatments.

When you enter the space, a mask must be worn, we will take your temperature using thermal scanner (whichever is more efficacious), patients submit s/s waiver (in addition to their other waivers) while checking in We will be wearing PPE (N95’s and face shields). All of our employees are checked for symptoms daily and must submit a sign, symptom, and travel questionnaire daily.

At TRU Form Skin Lab your treatment provider may be a Physician, Nurse practitioner, or Registered Nurse. Our specialists are highly trained medical professionals with a passion for aesthetics and treatment perfection. They are Board Certified with experience in Cosmetic plastic surgery, Dermatology, Aesthetics, and Family Practice. They believe that deep listening and excellent client care are the keys to bring forth the beauty within and help clients find their TRU FORM!

You will meet your service provider; your temperature will be taken, and you will be seated to fill out a quick 5 question symptom and travel COVID questionnaire on the intake tablet. You will then be asked to review your waivers and the treatment selected; your provider will assess and confirm or alter your treatment based on your treatment needs and desires. Your provider will confirm or update the treatment, and they will proceed to prepare you for treatment. Following your upgrade, you will be given post treatment therapy and instructions, verbal and written, and go on your way to slay!
