Booking Statement

  1. Book online, pay your $50 deposit, and the rest the day of your treatment.

  2. Payment options include the ability to pay a percentage of the cost of your treatment at different intervals until the time of your appointment; remember all fees for treatment are due at the time of your treatment.

  3. Cancellation must happen 24 hours before your appointment. Failure to cancel will result in an inability to book online or over the phone without submission of a non-refundable $100 deposit to be used at the time of future treatment.

Patient Assessment Statement

  1. All patients will be thoroughly assessed before any treatment.

  2. The provider reserves the right to deny or reschedule treatment that would be unsafe, as well as if the patient is physically or emotionally inappropriate for treatment.

Covid-19 Statement

  1. For the protection all our clients we have implemented the following policies in response to COVID-19.

  2. Clients and staff must wear a mask at all times in common areas,

  3. Clients will have their temperature checked upon entry using a thermal scanner,

  4. All patients submit “travel, signs, and symptoms” waiver (in addition to their other waivers) while checking you in.

  5. Our staff will be wearing PPE (N95’s and face shields).

  6. All of our employees are checked for symptoms and must submit a sign, symptom, and travel questionnaire daily.

TRU Form Skin Lab is committed to providing a website that is accessible to the widest possible audience, regardless of circumstance and ability. We aim to adhere as closely as possible to the Web Content Accessibility Guidelines (WCAG 2.0, Level AA), published by the World Wide Web Consortium (W3C). These guidelines explain how to make Web content more accessible for people with disabilities. Conformance with these guidelines will help make the web more user friendly to everyone. Whilst TRU Form Skin Lab strives to adhere to the guidelines and standards for accessibility, it is not always possible to do so in all areas of the website and we are currently working to achieve this. Be aware that due to the dynamic nature of the website, minor issues may occasionally occur as it is updated regularly. We are continually seeking out solutions that will bring all areas of the site up to the same level of overall web accessibility.
